Please prepare for the following before your upcoming appointment:
New Patients: Please fill out the appropriate new patient paperwork located on our forms page .
Ultrasound Visit:
Please arrive with a full bladder. You may bring (1) adult to your appointment. All approved visitors will be required to clear a health screening before entering our offices. Please review our Visitor Policy above with any questions.
Postpartum Visit:
Please review our Visitor Policy. As an exception, your newborn is allowed to accompany you in addition to (1) adult visitor. Any child with you must be in a stroller/carrier. No other children over the age of 2 are allowed in the office.
C-PAP Compliance Visit:
Your appointment should be scheduled 30 days after your initial visit for your C-PAP machine. Please bring your C-PAP SD card if your machine has one. If you are having issues with the settings on your machine, please bring the machine with you to your appointment for your doctor to review.
Pulmonary Function Test (PFT) Visit:
Please do not wear tight clothing to your appointment. If you use nebulizers/inhalers/bronchodilators please bring them with you to your appointment in case you need them after the test. Nebulizer and inhalers may not be used 24 hours before your PFT. Do not consume caffeine or stimulants 8 hours before the visit. Do not smoke within 4 hours before the visit. Do not eat or drink anything 2 hours before the visit. If you bring someone with you to your appointment they must wait in the waiting room during your test.
MDVIP Visit:
You must check in at the front desk in the main waiting room. If you are scheduled for your Annual Wellness Exam, please complete a copy of our privacy policy and financial policy before your visit.
MDVIP bloodwork requires fasting 8-10 hours before your appointment. You're welcome to take your medications with water within that time. Also, please avoid fish and fish oil products within 24 hours before your blood draw.
Pre-op Visit:
If your surgeon is requiring a pre-op clearance, please have them send us a copy of their clearance form to us by fax at 281-578-4912. You may also submit a copy of the form via our patient portal or by dropping it off at our front desk. Once we have the form, our office will contact you to schedule an appointment for your pre-op.
