Pre-Visit Instructions

Please prepare for the following before your upcoming appointment:

• Arrive at least 5-10 minutes before your scheduled appointment
• Masks are required to be worn if you are experiencing cold, flu, or allergy symptoms
• Bring a government issued I.D. and current insurance card(s)
• Bring a list of your current prescription medications with dosages
• If you need forms filled out for any reason (i.e. FMLA, home health, etc), please bring them with you
• Familiarize yourself with our policies and any unique appointment requirements below

New Patients: Please fill out the appropriate new patient paperwork located on our forms page .

• Due to our limited space, we are unable to accommodate large groups of people and strongly discourage more than 2 visitors per patient.
• Children are welcome but visitors 12 years and younger must stay by your side during the full duration of your visit.
• All visitors, including children, are required to follow the same safety protocol as our patients.
• Our clinic is a fragrance free office. Please do not wear strong perfumes or colognes in our office as they can trigger asthma attacks.
• Emotional support animals are not permitted inside our building. If you have an ADA protected service animal, please notify our staff when scheduling your appointment so that we can make appropriate accommodations.
• If you have questions about our policy, please feel free to call our office for further clarification.

Ultrasound Visit:

Please arrive with a full bladder. You may bring (1) adult to your appointment. All approved visitors will be required to clear a health screening before entering our offices. Please review our Visitor Policy above with any questions.

Postpartum Visit:

Please review our Visitor Policy. As an exception, your newborn is allowed to accompany you in addition to (1) adult visitor. Any child with you must be in a stroller/carrier. No other children over the age of 2 are allowed in the office.

C-PAP Compliance Visit:

Your appointment should be scheduled 30 days after your initial visit for your C-PAP machine. Please bring your C-PAP SD card if your machine has one. If you are having issues with the settings on your machine, please bring the machine with you to your appointment for your doctor to review.

Pulmonary Function Test (PFT) Visit:

Please do not wear tight clothing to your appointment. If you use nebulizers/inhalers/bronchodilators please bring them with you to your appointment in case you need them after the test. Nebulizer and inhalers may not be used 24 hours before your PFT. Do not consume caffeine or stimulants 8 hours before the visit. Do not smoke within 4 hours before the visit. Do not eat or drink anything 2 hours before the visit. If you bring someone with you to your appointment they must wait in the waiting room during your test.

MDVIP Visit:

Our space has moved! Please check in at the front desk in the main waiting room for directions. If you are scheduled for your Annual Wellness Exam, please complete a copy of our privacy policy and financial policy before your visit.

MDVIP bloodwork requires fasting 12 hours before your appointment. You're welcome to take your medications with water within that time. Also, please avoid fish and fish oil products within 24 hours before your blood draw. We will also ask for you to leave a urine sample during this visit.

Pre-op Visit:

If your surgeon is requiring a pre-op clearance, please have them send us a copy of their clearance form to us by fax at 281-578-4912. You may also submit a copy of the form via our patient portal or by dropping it off at our front desk. Once we have the form, our office will contact you to schedule an appointment for your pre-op.
