Pre-Visit Instructions


Checklist

Please prepare for the following before your upcoming appointment:

  • Complete COVID screening (below) approximately 24 hours before your appointment
  • Arrive wearing a mask at least 5-10 minutes before your scheduled appointment
  • Bring a government issued I.D. and current insurance card(s)
  • Bring a list of your current prescription medications with dosages
  • If you need forms filled out for any reason (i.e. FMLA, home health, etc), please bring them with you
  • Familiarize yourself with our policies and any unique appointment requirements below

New Patients: Please fill out the appropriate new patient paperwork located on our forms page .

COVID Screening

Please click on the appropriate button below to complete the COVID-19 screening questionnaire approximately 24 hours before your appointment. This is required before your visit to avoid possible cancellation or refusal of your appointment. If anyone is accompanying you to your appointment, they must also complete the COVID-19 screening questionnaire. Please see our Visitor Policy below for details on approved visitors.

If you are experiencing any COVID-19 symptoms, we kindly ask that you call our office to reschedule your visit for a Telemedicine Appointment.

Visitor Policy

As cases of COVID-19 continue to increase at an alarming rate, MCT is continuing to limit visitors across our office.

Patients will be limited to (1) adult accompanying you at your appointment. All approved visitors will be required to clear the COVID screening (above) before entering our offices. No children over the age of 2 are allowed in our office. If you have questions about our visitor policy, please call our office

If you have a(n)

Ultrasound Visit:

Please arrive with a full bladder. You may bring (1) adult to your appointment. All approved visitors will be required to clear a health screening before entering our offices. Please review our Visitor Policy above with any questions.

Postpartum Visit:

Please review our Visitor Policy. As an exception, your newborn is allowed to accompany you in addition to (1) adult visitor. Any child with you must be in a stroller/carrier. No other children over the age of 2 are allowed in the office.

C-PAP Compliance Visit:

Your appointment should be scheduled 30 days after your initial visit for your C-PAP machine. Please bring your C-PAP SD card if your machine has one. If you are having issues with the settings on your machine, please bring the machine with you to your appointment for your doctor to review.

MDVIP Visit:

You must check in at the front desk in the main waiting room. If you are scheduled for your Annual Wellness Exam, please complete a copy of our privacy policy and financial policy before your visit.

Pre-op Visit:

If your surgeon is requiring a pre-op clearance, please have them send us a copy of their clearance form to us by fax at 281-578-4912. You may also submit a copy of the form via our patient portal or by dropping it off at our front desk. Once we have the form, our office will contact you to schedule an appointment for your pre-op.
